ABSTRACT

Objective: To study the chemical constituents from Medicated Leaven (namely Massa Medicata Fermentata in Chinese). Methods: Compounds were isolated and purified from chloroform extract of Medicated Leaven by column chromatograph of silica gel, ODS, Sephadex LH-20, HPLC, and their structure were elucidated on the basis of spectral data and physicochemical property. Results: Eleven chemical compounds were identified as 5-pentacosyl resorcinol (1), 5-heptadecyl resorcinol (2), 5-heneicosyl resorcinol (3), 5-tricosyl resorcinol (4), (9S,10E,12Z)-9-hydroxy-10,12-octadeca-dienoic acid (5), trans-4-hydroxy-2-nonenoic acid (6), 9,10,11-trihydroxy-11(E)-octadecenoic acid (7), (9S,12R,13S)-9,12,13-trihydroxy-10(E)-octadecenoic acid (8), (9S,12S,13S)- 9,12,13-trihydroxy-10(E)-octadecenoic acid (9), (9S,12S,13S)-9,12,13-trihydroxy-10(E)-octadecenoic acid methyl ester (10), and 9,12(Z)-octadecadienoic acid methyl ester (11). Conclusion: All above compounds are all isolated from Massa Medicata Fermentata for the first time, and the chloroform extract of Massa Medicata Fermentata and compound 8 showed inhibitory effect against TNF-α generation of RAW 264.7 induced by LPS in a dose-dependent manner.

ABSTRACT

AIM To study the chemical constituents from Polygonum lapathifolium L..METHODS The ethyl acetate fraction of 80% ethanol extract of P.lapathifolium L.was isolated and purified by silica column,Sephadex LH-20 and ODS,then the structures of obtained compounds were identified by physicochemical properties and spectral data.RESULTS Fifteen compounds were isolated and identified as 2',4'-di-hydroxy-6'-mehtoxy-chaltone (1),2',6'-di-hydroxy-4'-methoxy-chaltone (2),2',3'-di-hydroxy-4',6'-di-methoxy-chaltone (3),kaempferol (4),quercetin (5),quercetin-3-O-α-L-rhamnoside (6),quercetin-3-O-α-L-rhamnoside (7),kaempferol-3-O-α-L-rhamnoside (8),kaempferol-3-O-β-D-glucoside (9),quercetin-3-O-β-D-galactoside (10),quercetin-3-O-β-D-glucoside (11),kaempferol-3-O-β-D-galactoside (12),quercetin-3-O-(2"-O-galloyl)-rhamnoside (13),quercetin-3-O-(2"-O-galloyl)-glucoside (14),quercetin-3-O-[(6"-O-transferuloyl)-β-D-galactoside] (15).CONCLUSION Compound 3-8 are isolated from this plant for the first time.

ABSTRACT

<p><b>BACKGROUND</b>Fingertip defect significantly affects the appearance of the hand. The aim of this research was to evaluate the clinical effect of aesthetic fingertip reconstruction with partial second toe transfer.</p><p><b>METHODS</b>Between July 2005 and December 2008, 17 patients underwent aesthetic fingertip reconstruction with partial second toe transfer. The mean size of fingertip defects was 0.7 cm (ranging from 0.5 - 1.1 cm). Anastomosis was performed between one dominant plantar digital artery and one proper digital artery, between two dorsal digital veins of the toe and two dorsal digital veins of the finger, respectively, for the reconstruction of blood supply to the reconstructed finger. Two plantar digital nerves of the toe were then sutured to two proper digital nerves of the finger for the restoration of fingertip sensation.</p><p><b>RESULTS</b>All reconstructed fingers survived completely. The follow-up period ranged from 6 to 24 months, and the appearance of the reconstructed fingertip was similar to that of the normal fingertip. Two-point discrimination of the reconstructed fingertip was (7.8 ± 1.3) mm. The recovered pinch strength of the reconstructed finger was about (89.0 ± 5.1)% of that of the normal finger. Donor site healed well without complications.</p><p><b>CONCLUSIONS</b>Partial second toe transfer is an ideal aesthetic reconstruction method for fingertip defects; it can not only achieve a satisfactory appearance of the fingertip, but can also obtain excellent sensory and motor functions.</p>

@#ObjectiveTo investigate the encountered life events in patients with tuberculosis within 1 year before the attack.MethodsThe encountered life events within 1 year before the attack were investigated in 85 patients with tuberculosis, the life event frequency (LEF) and life event intensity (LEI) were calculated respectively, and the life events were compared with those within 1 year in 55 healthy subjects.Results71 cases (83.5%) of the 85 patients with tuberculosis had encountered different life events within 1 year before the attack, which was significantly different from that in the control group (9 cases, 16.4%) (χ2=12.351,P<0.01 ). The LEF, negative LEF, average LEI, average negative LEI in the tuberculosis group before the attack were obviously higher than those in the control group (P<0.05~0.01), but there were no significant differences in the positive LEF and average positive LEI between the two groups (P>0.05).ConclusionThe encountered life events are closely correlated with primary tuberculosis, so more mental cares should be paid to the patients.

ABSTRACT

<p><b>OBJECTIVE</b>To study the clinical significance of changes of serum myocardial enzymes in patients with acute carbon monoxide poisoning.</p><p><b>METHODS</b>To determine the dynamic changes of the activity of myocardial enzymes and ECG in 62 patients with acute CO poisoning.</p><p><b>RESULTS</b>In patients with acute CO poisoning 5 kinds of myocardial enzymes begin to increase within 24 hours, the activities of aspartate aminotransferase (AST), creatine phosphokinase (CPK), lactic dehydrogenase (LDH), alpha-hydroxybutyrate dehydrogenase (alpha-HBDH), CPK isoenzyme (CK-MB) were (20.2 +/- 12.3), (151.6 +/- 91.8), (146.8 +/- 50.4), (154.8 +/- 47.7), (13.8 +/- 8.1) U/L respectively, while those in control group were (12.1 +/- 6.7), (90.6 +/- 17.3), (118.7 +/- 13.5), (89.9 +/- 27.9), (5.9 +/- 3.3) U/L respectively. There was significant difference between two groups (P < 0.01); 3 d later, the activities of 5 enzymes were still increased [(21.3 +/- 12.3), (105.8 +/- 51.4), (144.8 +/- 51.4), (159.8 +/- 35.4), (16.2 +/- 9.1) U/L respectively]. 7 and 12 d later, the activities of alpha-HBDH and CK-MB were still higher than those of control (P < 0.01). LDH(1) and LDH(2) increased to peak value in 24 h after poisoning (35.3 +/- 5.8), (43.8 +/- 5.7) U/L vs (24.8 +/- 3.9), (36.9 +/- 4.3) U/L, P < 0.01. The abnormal rate of serum LDH(1) was 78.7%, LDH(2) 58.3%, LDH 45.2%, CK-MB 37.1%, alpha-HBDH 33.6% and the abnormal rate of ECG was less than 10%.</p><p><b>CONCLUSION</b>Acute carbon monoxide poisoning may cause myocardial injury. Determination of serum myocardial enzymes may contribute to showing myocardial injury, early diagnosis and treatment, results of treatment and prognosis.</p>
